Online USAV Region Membership Registration

1. If you were registered in the Lone Star Region after November 1, 2004, your information is already in the database.  USAV members registered with others regions anytime after November 1, 2004, check with the Region Office before starting your registration.
2. Once you are ready to get started, click the appropriate link.
3. Follow the on-screen instructions.  If you need help, do not hesitate to call the Region Office (210) 945-4365.  If you have a problem getting through to the Region Office you may try Dick Kappel at (830) 606-4621.
4. After the system asks you to change your password, on the left-hand side you will need to click on "Renew On-Line" and continue to follow the instructions.
5. We will not have on-line payments for the 2006/2007 season.
6. The following forms need to be printed at the completion of the on-line registration process:
   1. Junior Members, you must print the Waiver & Release of Liability and Medical Release Form.  They must be signed and given your club director.  (Make sure your parent or guardian also signs the form)
   2. Coaches, Team/Club Rep, Trainer, Chaperone, you must print the Waiver & Release of Liability and Background Screening Form (when required).  These forms must be signed and given to your club director.
   3. Adult Players or Officials (not affiliated with a Junior Club),you must print the Waiver & Release of Liability.  You may give the signed Waiver to your adult club director.  Or, you may send the Waiver, along with your registration fee to the Lone Star Volleyball Region,  1608 Canyon Oak; Schertz, TX 78154.

Club directors should collect all forms and fees from their members and send to the office as soon as possible.  No one will be registered until the forms and fees are received in the office.  Unpaid members will not participate in any sanctioned even (including) practices.  No exceptions!
